OFFICIAL TRANSCRIPT AND CERTIFICATE OF ENROLLMENT

HACU requires that an official transcript be submitted from the institution where you are currently enrolled.

Please note that your official transcripts, along with your Certificate of Enrollment must be received by HACU before your application is considered complete. Applications must be complete to be considered for any award.

Download the CERTIFICATION OF ENROLLMENT AND CLASS LEVEL FORM and have it completed by an academic advisor, dean, registrar or other school official.

Please mail the completed certificate of enrollment form, along with your official transcripts, by June 12, 2009 to:

DESIGN

1. Students must submit one original design of a suit for a job interview. The suit should be designed to fit you.
2. The design should be an illustration that measures 11x14 in a PDF or JPEG format. The PDF or JPEG should not exceed 8MB. E-mail final designs to designyourfuture@hacu.net or mail on a CD to the address above along with the certificate of enrollment and class level form, If e-mailed, please provide students first and last name in the subject line.
3. You may color your illustrations, using the following:

• Watercolor
• Pen and ink
• Felt tip pen
• Pastels
• Colored pencils
• Black and white/Flats/Croquis/technical sketches

All designs submitted to HACU remain the property of the student however they will NOT be returned. Please make sure and keep a copy for your personal records.

Failure to answer BOTH questions below will render your application ineligible.

STUDENT CERTIFICATION

I hereby agree that Gap Inc., Latina Stlye Magazine, and HACU shall have, without further obligation to me, the right to use my name, voice, likeness and essays, and/or photographs and designs and biographical information for advertising and promotional purposes in conjunction with this and similar promotion without notice to me and without further compensation unless prohibited by law.

It is understood and agreed that this is a complete RELEASE and DISCHARGE of all claims and rights of the undersigned against the aforementioned parties to be released, and that no action will be taken by or on behalf of the undersigned with respect to any such rights, it being understood that this release shall be binding upon the heirs, executors, and administrators of the undersigned.

I also give my authorization, permission and consent to HACU to contact and receive information for any institution, agency, college/university and/or persons for the following purposes:

1. to verify any of the information I have provided or may provide to HACU, and/or
2. to obtain any other information concerning my scholastic status in order to assess and monitor my qualifications and or continued eligibility for this or any other award.

The authorizations, permissions and consents I have herby given shall remain in full force and effect unless I revoke this consent in writing by letter addressed to HACU’s Student Services Department.

I hereby verify that all of the information I have provided is complete and accurate to the best of my knowledge. I further acknowledge and agree that any incomplete or false information may result in my application not being considered or in the termination of any award which may be granted.

Final Submission Notice

Please note that once you agree to these terms and submit the application you will not be able to edit, change or update this application. Therefore, please take a moment to double check all of your personal information and make sure there are no grammatical mistakes in the essay.
